    CKD Stage1~2

    Basic Condition: On March 20th, 2012, she entered into our hospital for medical treatment attributes to intermittent macroscopic hematuria with diseased course for 5 years. Intermittent macroscopic hematuria existed 5 years ago puts down to cold and fever that patient was then diagnosed by local hospital with IgA Nephropathy with urine protein (++), occult blood (+++) and renal biopsy. (without specific result.) Then she was treated with irbesartan instead of losartan potassium to control blood pressure of 139/100mmHg, which is stopped in 2010 with normal blood pressure level for several times. Now the BP maintains normal. While gross hematuria existed for a couple of times after 3~4 days of cold during these 5 years, which is accompanied with slight edema of lower limbs and foamy urine. Then she was treated with homeopathy (without details) and acupuncture therapy while received limited efficacy. Besides, she was not applied with hormone or immunosuppressive agent. Two years ago, arthralgia appeared occasionally, which can be eased through rest. A week ago, she caught cold again with hyperpyrexia, cough, expectoration and gross hematuria. There is no other past medical history, drug allergy history or family history of kidney disease.

    Hospital Examination: BP: 130/80mmHg with slight pitting edema of lower limbs. There is no other positive sign.

    Laboratory Examination: Blood Routine: Hgb 145g/L, WBC 10.8*10^9, PLT 336*10^9/L

    Electrolyte: K 4.4mmol/L, Ca 2.53mmol/L, P 1.36mmol/L

    24-Hour Urinary Protein Quantity: 1.95g/24h

    Plasma Albumin: 40.8g/L (Reexamination with 33.6g/L on April 1st)

    Blood Lipid: TG 2.07mmol/L, CHOL 5.93mmol/L, LDL-C 3.92mmol/L

    Renal Function: Cr 110umol/L,Bun 5.3mmol/L, UA 448umol/L, cystatin c 1.55mg/L, Beta 2 micro serum globulin of 1.55mg/L, Ccr of 65.28ml/min.

    Lymphocyte Subpopulation: CD3+ 2151/ul, CD3+CD8+ 946/ul

    Urine Microalbumin: 1371mg/L, Microglobulin Urine Alpha 1 of 113.4mg/L, urine osmotic pressure of 260mOsm/(kg.H2O)

    Heart Color Ultrasound: small quantities of tricuspid regurgitation

    Abdominal Sufficiency: right kidney of 80*38*36mm, left kidney of 81*46*38mm. Ambiguous outline of bilateral kidneys with rough envelope, increased echo of bilateral renal parenchyma, rough borders of cortex and medulla and dispersive structures of collecting system.

    Treatment condition: Micro-Chinese Medicine Osmotherapy is applied primarily with oral taking Chinese herbs, which is accompanied with tripterygium glycosides, allopurinol and so on. Edema is remedied after 1 month of medical treatment with creatinine 107umol/L and 24-hour urinary protein quantity of 0.7g. Now she has better appetite and sleep.
