    Why Does Low GFR Level Exist in CKD Stages

    Low GFR Level, CKD StagesDo you know that patients may have already suffered from asymptomatic period for a long time before the appearance of various obvious symptoms of CKD? Actually, it is hard to diagnose early condition accurately without a series of related tests and GFR is one of the effective indexes which can clearly reflect the progress of CKD. However, why does Low GFR Level exist in CKD Stages?

    As we all know, there are five CKD stages which are divided on the basis of different GFR, which shows varying degrees of renal damage and filter capacity decline except stage Ⅰ which presents slight renal damage with normal or little increased GFR.

    When GFR is 60~89ml/min, which indicates renal damage with slight GFR decline that patients need to be estimated, thus slowing down the progress and reducing the risk of CKD, while 30~59ml/min in CKD stage Ⅲ means moderate low GFR level that patients should be aimed at easing symptoms and treating a mount of complications. Severe low GFR level exists when it drops to 15~29ml/min, which needs comprehensive treatment and dialysis preparation. What’s worst? Patients finally enters into End Stage Renal Disease when GFR is under 15ml/min, which further leads to End Stage Renal Failure and replacement therapies are inevitable.     One question should be answered first before asking about the causes of low GFR level in CKD stages, that is how does the glomerular play its role. Normally, glomerular is able to filter substances selectively through glomerular filtration membrane which includes epithelial cell, basilar membrane and endothelial cell. Barrier dysfunction exists when permeability change of glomerular basement membrane appears with renal injuries which is caused by CKD, and mechanical barrier and charge barrier are damaged that can not purify the blood normally, thus leading to low GFR level.

    In a word, Low GFR level exists because of the damage of renal intrinsic cells, only by repairing renal injuries and restoring renal functions can we regulate low GFR level fundamentally.
