    Diabetic Nephropathy: What Stages Are You in?

    Stage 1: High GFR period with hypertrophic kidneys

    Stage 2: Conventional proteinuria period

    GFR is higher than normal, which mainly behaves as incrassated glomerular basement membrane and increased mesangial matrix. Urinary albumin excretion (UAE) rises after exercise, which gradually returns to normal with rest. It is worth noting that patients with Diabetic Nephropathy will stay steadily in this condition for a relatively long time with well glycemic control.

    Stage 3: Diabetic Nephropathy of early stage, known as continuous slight proteinuria period

    GFR starts to decline to normal, which indicates renal pathological changes of nodular nidus of glomerulus and hyaline lesions of arteriole. UAE continuously rises to 20~200μg/min with slight proteinuria, which can be eased with ACEI or ARB so as to slow down the progression of Diabetic Nephropathy.

    Stage 4: Clinical period of Diabetic Nephropathy

    K-W nodule appears as typical pathological change, which leads to heavy continuous proteinuria (UAE>200μg/min) or proteinuria of more than 500mg/d that Nephrotic Syndrome appears to 30% of patients with continuous GFR decline. The characteristic in this period is that proteinuria won’t decline with GFR.

    Stage 5: End Stage Renal Failure period

    Patients with Diabetic Nephropathy have less than 10ml/min GFR while protein in urine reduces with glomerular sclerosis. Various systemic symptoms and complications appear, which requires for dialysis.
